Isotretinoin, spironolactone and steroid injections

Cystic acne treatment

Deep, painful cysts and nodules need a dermatologist: retinoids with benzoyl peroxide, doxycycline, the birth control pill or isotretinoin for scarring acne, and a triamcinolone shot for one large cyst.

What cystic acne is

Cystic acne (nodulocystic acne) is acne that sits deep in the skin and forms hard, painful lumps. It means the lumps are bigger than ordinary pimples, can join together and can leave scars if severe acne goes untreated. The cause of a cyst is a clogged follicle deep in the skin, where trapped oil, dead skin cells and bacteria set off a strong immune reaction.

Hormone changes that make the skin oilier are the usual trigger: puberty, periods, pregnancy, birth control pills and stress. Acne also tends to run in families.

What to ask your dermatologist about

Topical Medications

A topical retinoid (adapalene, tretinoin) with benzoyl peroxide has a strong recommendation. Adapalene 0.1% gel and benzoyl peroxide are sold over the counter; ask about prescription retinoids if they are not enough.

Oral Antibiotics

Doxycycline is the antibiotic pill with a strong recommendation. The guideline says to limit antibiotic pills and always pair them with a topical.

Hormonal Treatments

For women, both carry a conditional recommendation. In the SAFA trial of 410 women, spironolactone beat a placebo at 12 weeks and the gap grew by 24 weeks.

Isotretinoin

The guideline strongly recommends isotretinoin for severe acne, acne that scars and acne that fails standard treatment. Everyone who takes it, men included, must be registered with iPLEDGE.

Corticosteroid Injections

A low dose steroid such as triamcinolone goes straight into one large, inflamed lesion. The guideline lists it as good practice for larger lesions, and an open label study found no adverse events.

In-Office Procedures

In-office procedures are treatments a dermatologist does in the clinic, next to your medicine. Ask whether they fit you.

Chemical Peels & Light Therapies
Chemical skin peeling, or photodynamic therapy: a chemical that blue light activates goes on the skin before a light session.
Scar Treatments
Scars left by acne can be removed with dermabrasion; ask whether it fits your skin.

Temporary At-Home Relief

Temporary at-home relief is what you can do tonight while a treatment starts to work. It does not replace a dermatologist.

Cold Compresses
A clean, cold compress held on the spot can feel soothing. Wrap ice in a cloth instead of putting it on bare skin.
Gentle Cleansing
Wash twice a day with a gentle cleanser. Don't scrub: it irritates the skin and can make acne worse.
Gentle Care
Use skincare and makeup labeled non-comedogenic (won't clog pores) and take makeup off at night.
Do Not Pop or Pick
Squeezing a cyst can lead to infection, slower healing and scars.

When to see a dermatologist

See a doctor or dermatologist if:

  • you have deep, painful cysts or nodules
  • your spots are leaving scars
  • self-care and pharmacy products have not helped after several months
  • the skin around a spot turns hot, swollen and red
  • acne is affecting your mood

Your questions, answered plainly

What is the main cause of cystic acne?

A pore gets plugged with oil and dead skin cells deep in the skin, your immune system reacts to bacteria trapped inside, and you get a hard, sore lump. Hormone changes that make skin oilier are the big trigger, and it tends to run in families.

What antibiotic clears up cystic acne?

Doxycycline has the strongest backing of the antibiotic pills in the 2024 US guideline. It is meant as a short course next to a cream such as benzoyl peroxide, never on its own. Ask your doctor whether it suits you.

How do you get rid of a cystic pimple with no head?

Leave it alone: squeezing can lead to infection, slower healing and scars. Try a cold compress for comfort, and if it is big and painful, a dermatologist can inject a low dose steroid straight into it.

What shrinks cystic acne fast?

For one large cyst, a steroid injection from a dermatologist is the quickest route. Creams and pills treat the whole face but often need 6 to 8 weeks.

How to tell if acne is hormonal or bacterial?

It is rarely one or the other: trapped bacteria are part of how most pimples form. Hormonal clues are breakouts on the lower face and jawline and flares that follow your menstrual cycle. For a read on your own skin, the photo analysis looks at your pattern.
